As part of our accreditation process with the Council on Accreditation (COA), we invite all stakeholders, including clients, families, staff, board members, and community partners, to share feedback with us or directly with COA.
If you would like to report a concern about any aspect of our services, you may do so confidentially using the link below. Your input goes to our peer review team and helps support a thorough, independent evaluation of our organization.
Submit a Concern Directly to COA
COA reviews all submissions to determine if they indicate a potential violation of standards. If so, those areas will receive additional focus during the site visit. Thank you for helping us uphold a culture of safety, integrity, and continuous improvement.
