Admitting a Child

Black Mountain Home is licensed by the North Carolina Department of Health and Human Resources. The Home accepts both private and Department of Social Services placements.

Limited scholarships are available for private placements after considering financial resources of both the child and the family. Children admitted to the Home must be able to function in a public school setting and provide minimum self-care. Children must generally be between the ages of 5 and 17.

Due to the openness of the campus and the individual freedom that is encouraged, Black Mountain Home for Children, Youth & Families cannot admit:

  • Children who cannot function in public schools.
  • Children who are violent/school-banned felons.
  • Children expecting total emancipation soon.
  • Persistent drug and alcohol abusers.
  • Children with a history of arson.
  • Recent and persistent runaways.
  • Children with recent suicide attempts not treated with psychiatric hospitalizations.
  • Private placements due to family conflicts where the family is not in active counseling.

Black Mountain Home for Children, Youth & Families uses the standard NC form.

Checklist for Admissions

The following items are required for admission to the Home. These documents must be submitted to the Support Services Department before a child can be admitted:

  1. A completed state application form (required prior to admission).
  2. A completed medical exam and record form, including a doctor's examination. (Medical forms are part of the application packet.) A TB test (with results) is mandatory if the child has not received one within the past year.
  3. A certified copy of the child's birth certificate. This can be obtained from the Register of Deeds office of the county where the child was born.
  4. Immunization record, including all shot dates. All immunizations must be up-to-date before admission to the Home. This is required by the public schools.
  5. School affidavit form (required prior to admission visit). Must be notarized.
  6. School records, including transcript and any achievement test results, if available.
  7. A copy of any psychological testing results and/or psychiatric evaluations.
  8. A copy of custody papers or court order (if parent is not the guardian).
  9. A completed packet of signed and notarized release forms, which include release of information, travel, educational decisions, picture, visitation, permission to treat emergencies (medical, dental, psychiatric).
  10. Original Social Security card of the child. If child does not have a card, he/she must apply for it prior to admission.
  11. Proof of insurance (Medicaid or private).
  12. Records of any juvenile or family court involvement.
  13. Face to face interview of child being considered in order to document the child's understanding of the purpose of a need for out-of-home care and residential services.
  14. Information shared about BMH's program and to provide written information (copy of the "Code of Living" for both child and guardian).
  15. Other information shared about the child's needs at admission.
  16. Dates for 30, 60, and 90 day reviews of service plans. (on admission)
  17. Financial agreement with the guardian. (on admission)
  18. A copy of the D.S.S. case plan.
